Update Your Phone Number or Address

To update your phone number, home address, or mailing address, choose one of the following options:

 

Option 1 - Complete a Change Form and send it to Tempus via email (MAFMS@Tempusunlimited.org) or fax (800-359-2884), or drop the form off at a Tempus office location.

 

Option 2 - Send Tempus FI a letter asking to update your contact information. The letter must include your new phone number and/or address. Send the letter to Tempus via email (MAFMS@Tempusunlimited.org) or fax (800-359-2884), or drop the form off at a Tempus office location.

 

Option 3 - If you are a Consumer or Surrogate, ask your Personal Care Management (PCM) Agency to update your contact information directly in Tempus’ Enrollment System.

* Please ensure the email address you provide to Tempus Unlimited is not being shared with another person in the PCA program. Each FI/EVV user needs an email account to access the system.

To update your email address, fill out the form below and click “Continue” to look up your account. Once you look up your account, you will be able to update your email:
